7. Complete the credit card information section by selecting the type of card (e.g., Visa or Mastercard), entering the card number, expiration date (month and year), and CVV code.
8. Authorize Penske Truck Leasing Co., L.P. to process payment by signing the form. Provide the date of signature.
9. After completing all sections, save your changes. You may then download, print, or share the form as needed for submission.
