An Individualized Family Service Plan (IFSP) documents and guides the early intervention process for children with disabilities and their families. The IFSP is the vehicle through which effective early intervention is implemented in ance with Part C of the Individuals with Disabilities Education Act (IDEA).

The New Jersey Early Intervention System (NJEIS) implements New Jersey's statewide system of services for infants and toddlers, birth to age three, with developmental delays or disabilities, and their families.

As a child approaches 3 years of age, the service coordinator will help with transition from early intervention to a preschool program which may be provided by their local school district, and/or other service providers, based on the child's needs.

System of Payment and Family Cost Participation The cost of Early Intervention services for a child is determined by their family income. Families whose income is less than three times the federal poverty level will receive Early Intervention Services at no cost.

In technical terms, children are considered eligible when they are experiencing a developmental delay that meets these criteria: 1.5 standard deviation below the mean in each of two functional developmental areas (scored below approximately 90% of children their age); OR.

Early Intervention Services assists developmentally delayed children until their third birthday. Families of children 3 and older can receive referrals for assistance from Project Child Find.
