If additional space is required, attach a separate sheet of paper. Submit Application and Supporting Documentation to VA by: Mail: to NCA Intake Center, P.O. Box 5237, Janesville, WI 53547; or. Fax: to the National Cemetery Scheduling Office at (855) 840-8299.

Use VA Form 40-10007 to find out in advance if you can be buried in a VA national cemetery or when you're applying on behalf of a Veteran or service member.

Under the current regulations, VA pays for burial and funeral expenses on a reimbursement basis, which requires survivors to submit receipts for relatively small one-time payments that VA generally pays at the maximum amount permitted by law.
