8. Indicate your account type by checking either 'Checking' or 'Savings.'
9. Enter the bank transit routing number, which is the nine-digit number typically found on your checks. Follow this with your bank account number.
10. If you selected 'CHANGE' earlier, fill out the old bank transit routing number and the old bank account number that need to be deleted.
11. Sign and date the form in the 'Authorization' section, ensuring your printed name and title are clearly noted as well.
12. After filling out the form, review it carefully for accuracy. You can then save your changes, download, print, or share the completed form as needed.
