All Commanders who exercise authority over an individual who is a member of the Armed Forces, or other person designated by such a commander to receive protected health information in order to carry out an activity under the authority of the Commander.

The only acceptable proof of profile for a Soldier to produce is a Department of the Army Form 3349, Physical Profile, not a sick call slip. The profile is honored only if the individual has the document on hand and can present it when asked.

If a profile is needed the Soldier must complete a profile request packet and send packet to usarmy.usarc.usarc-hq.mbx.armmc@mail.mil (AR-MMC mailbox). It is also the Soldiers responsibility to report medical conditions that effect medical readiness to their commander.
