7. Complete the representative declaration section, where the representative must confirm the accuracy of their information, agree to represent you, and understand the disclosure of their personal information.
8. Both you and your representative must then sign and date the declarations at the end of the form, confirming that all information provided is truthful and complete.
9. After completing the form, review all entries for accuracy. Once confirmed, you can save changes, download, print, or share the form according to your needs.
