2. Fill out SECTION 1: CONTACT INFORMATION. Include your last name, first name, middle name, TUid, current mailing address (city, state, and zip code), telephone number, and your Temple email address.
3. Complete SECTION 2: PERSONAL INFORMATION. Indicate your citizenship and ethnicity by selecting the applicable options. If your ethnicity is not listed, please specify.
4. In SECTION 3: ACADEMIC INFORMATION, provide your field of study at Temple University, as well as the term and year you were admitted to the Ph.D. program. Mention whether you have taken the comprehensive exams or indicate that you have not yet taken them.
5. Move to SECTION 4: PROPOSED SUMMER RESEARCH EXPERIENCE. Enter the title of your proposed research and the name of your faculty mentor.
6. Prepare and attach the required documents as indicated in SECTION 5: REQUIRED ATTACHMENTS. This includes a detailed plan for the mentored summer research experience and a cover letter from your faculty mentor supporting your application.
7. Once you have completed all sections and attached the necessary documents, save your changes. You can then choose to download, print, or share the completed form as needed.
