There are many types of schemas, including object, person, social, event, role, and self schemas. Schemas are modified as we gain more information. This process can occur through assimilation or accommodation.

The Five Schema Domains Defined Abandonment/Instability. Mistrust/Abuse. Emotional Deprivation. Defectiveness/Shame. Social Isolation/Alienation.

Case conceptualization includes: information regarding the client's problem, the past situations that shaped the person's problem, the current situations that maintain this problem, the short- and long-term therapy goals and developing an evidence-based treatment plan.

This individual case formulation helps to understand the patient's current symptoms and interpersonal problems in the light of their developmental context. During the therapy process all arising problems are linked to this individual case formulation and therapeutic interventions are on the basis of the formulation.

Schema modes are organized into four categories: child modes, coping modes, parent modes, and the Healthy Adult mode. Modes can be adaptive (the Healthy Adult and the Happy Child mode) or maladaptive (every other mode).

These elements are described in terms of eight P's: presentation, predisposition (including culture), precipitants, protective factors and strengths, pattern, perpetuants, (treatment) plan, and prognosis. Presentation refers to a description of the nature and severity of the client's clinical presentation.
