Filling out the Case Log is an essential step for documenting surgical procedures and demonstrating your experience as a surgical first assistant. This guide provides clear, step-by-step instructions to help you accurately complete the log online.
Follow the steps to effectively complete your Case Log online.
- Press the ‘Get Form’ button to retrieve the Case Log and open it for editing.
- Begin by entering your name in the designated field at the top of the form.
- Next, provide the appropriate date range for the surgeries you are recording.
- You will need to document a total of 200 cases, organized by specialty. Ensure you include 75 cases in general surgery, 75 in one specialty area, and 50 in additional specialty areas.
- For each surgical case, fill in the case number, starting with 1 and continuing sequentially.
- In the 'Procedure date' field, enter the date when the surgical procedure took place.
- Specify the type of surgical procedure performed in the corresponding field, such as 'Laparoscopic Cholecystectomy'.
- In the 'Surgical Assisting Detail' section, describe your actions during the procedure using clear action verbs, for example, 'positioned', 'draped', 'trocar insertion', etc.
- Finally, provide the name of the facility where the procedure was performed.
- After completing all fields for each case, you can save your changes, download the document, and, if needed, print or share the form.

Residents can access the Case Log System directly at .acgme.org/connect. Program leaders and administrators can access Case Log reports and preview the logging screens on the Case Log tab when logged into ADS.
