The Holliday-Segar nomogram approximates daily fluid loss, and therefore the daily fluid requirements, as follows: 100 ml/kg for the 1st 10 kg of wt. 50 ml/kg for the 2nd 10 kg of wt. 20 ml/kg for the remaining wt.

When prescribing IV fluids, remember the 5 Rs: Resuscitation, Routine maintenance, Replacement, Redistribution and Reassessment.

ATLS continues to support the use of a 3-for-1 rule (3 mL of crystalloid should be used as replacement for every 1 mL of blood loss), but also encourages frequent reassessments if large amounts of crystalloid are not providing adequate resuscitation. In anesthetic practice, this formula has been further simplified, with the hourly requirement referred to as the “4-2-1 rule” (4 mL/kg/hr for the first 10 kg of weight, 2 mL/kg/hr for the next 10 kg, and 1 mL/kg/hr for each kilogram thereafter.

The 4 – 2 – 1 rule for maintenance IV fluid therapy (Normal Saline or Ringer's Lactate): 4 ml/kg/hr for the first 10kg of body mass. 2 mg/kg/hr for the next 10 kg of body mass. 1 mg/kg/hr for body mass beyond 20kg.
