9. Have the Site Supervisor complete the 'IN KIND Donations by HOST AGENCY' section. They should enter the in-kind contribution based on the supervisor's wage and hours spent supervising.
10. Finally, ensure that the host agency's supervisor signs and dates the form to confirm the reported information before submitting.
