The A4td form serves as a participant timesheet for the Associates for Training & Development’s Senior Community Service Employment Program. This guide will help you navigate the process of filling out the form online with ease and accuracy.
Follow the steps to effectively complete the A4td form online.

- Begin with entering your full name in the 'Participant Name' field. Ensure that this information is accurate to ensure proper identification.
- Next, provide the name of the hosting agency in the 'Host Agency Name' section. This identifies where you are gaining your training experience.
- Fill in the dates for each day worked during the payroll cycle starting from Monday through Saturday. Use the format MM/DD/YYYY.
- Calculate the total hours worked for the period indicated and enter this number in the 'TOTAL HOURS' section.
- In the 'HOST AGENCY TRAINING HOURS' area, document the hours earned through training along with any 'OFF-SITE TRAINING HOURS' that may apply.
- In the explanation field for off-site training hours, provide a brief description of the activities undertaken during these hours, particularly for Column C.
- Sign and date the form in the appropriate spaces to certify that all reported hours are accurate, excluding lunchtime.
- Have the Site Supervisor complete the 'IN KIND Donations by HOST AGENCY' section. They should enter the in-kind contribution based on the supervisor's wage and hours spent supervising.
- Finally, ensure that the host agency's supervisor signs and dates the form to confirm the reported information before submitting.
- Once you have completed all sections of the form, you can save any changes made, download, print, or share the form as necessary.
