In the absence of death, brain injuries are often one of the most devastating and long-lasting consequences of strangulation. Victims of intimate partner violence may also experience traumatic brain injuries (TBIs) from blows to head and other assaults. (Prosser, Grigsby, & Pollock, 2018; Valera & Kucyi, 2017).

Forensic nurses document patient injuries through tools including cameras, measuring tapes, fluid swabs, rape kits, and a high-powered light that can reveal hard-to-see bruises and fluids like semen, urine, or saliva.

The Strangulation Toolkit provides the forensic nurse with detailed guidance on assessment techniques, documentation, and evidence collection for this patient population.

Non-fatal strangulation and non-fatal suffocation often leaves minimal or no visible injury on the victim. The absence of evidence of injury is therefore not conclusive on the question of whether there is sufficient evidence to prosecute or not.

Strangulation can cause brain damage, pneumonitis, miscarriage, heart attacks, and delayed death, days or even weeks after the assault. In 50% of reported cases, this level of damage can be achieved without leaving noticeable bruises on the outside of the neck.

The word should be given its ordinary meaning which is the obstruction or compression of blood vessels and/or airways by external pressure to the neck impeding normal breathing or circulation of the blood. This offence applies where strangulation is non-fatal and does not result in death of the victim.

However, if the pressure is immediately released, consciousness will be regained within ten seconds. To completely close off the trachea (windpipe), three times as much pressure (33 lbs.) is required. Brain death will occur in 4 to 5 minutes, if strangulation persists.
