You must keep a running daily log of all your tip income. You can use Publication 1244, Employee's Daily Record of Tips and Report to Employer, to record your tip income for one year. Publication 1244 includes Form 4070, Employee's Report of Tips to Employer, and Form 4070A, Employee's Daily Record of Tips.

Tip income is taxable and must be reported | Internal Revenue Service.

Employers with tipped employees have payroll tax withholding, payment, and reporting responsibilities for tips that are considered taxable compensation. Some employers will qualify as large food and beverage establishments and may be subject to special tip allocation and reporting rules.

Add lines 11 and 12 together, and enter the total on Line 13. This is the additional Social Security and Medicare tax you need to pay for the tips you didn't report. You must also enter the figure on Line 5 of Schedule 2 of your Form 1040 tax return.
