- For Item 5, enter the year(s) of the tax returns you are requesting. For fiscal year filers, this will be the period ending date. If you require more than five periods, additional forms will need to be completed.
- In Item 6, provide your current street address. If this address is the same as the one above, simply enter 'Same' for Item 7.
- For Part III, remember that the form must be signed by the taxpayer for the release to be valid. If you are not the taxpayer, include a copy of your authorization.
- Finally, send the completed form to the PA Department of Revenue, Central Records Division, at the specified Harrisburg address. Allow four to six weeks for processing.
