Employers in California that have establishments meeting one of the requirements below are required annually to electronically submit Form 300A injury and illness data: All establishments with 250 or more employees, unless specifically exempted by section 14300.2 of title 8 of the California Code of Regulations.

Covered establishments must submit information from their Form 300A by March 2nd of the year after the calendar year covered by the form. See answers to more frequently asked questions on the rule.

You must post the Summary only — not the Log — by February 1 of the year following the year covered by the form and keep it posted until April 30 of that year. How long must you keep the Log and Summary on file? You must keep the Log and Summary for 5 years following the year to which they pertain.

You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.

It simply provides a totals for the number of cases, number of days, and illness/injury types, broken out by category. Employers have to keep all these records (the OSHA 300 log, OSHA 300a summary, and Form 301 incident reports) for five years.
