- Review all sections for accuracy before your employer completes and signs Section A and you sign Section B. Ensure the dates are correctly filled out.
- Submit the completed claim form. You can send it via mail, email, or fax to MERT Administration. Make sure to include any required documentation such as the Separation Certificate.
- After submission, you can save your changes or download a copy for your records. If required, print the form for your personal files.
