Follow the steps to successfully complete your Casting Form.

1. Press the ‘Get Form’ button to access the Casting Form and open it in your preferred online editor.
2. Begin by entering the date in the designated field to indicate when you are submitting the form.
3. If you are an ACTRA member, input your membership number in the specified box.
4. Fill in your name, birth date, and age. Ensure that all information is correctly spelled and formatted.
5. Provide your ethnic origin in the corresponding field as this information may be relevant for casting purposes.
6. Input your Social Insurance Number (SIN) in the required area, ensuring accuracy for identification.
7. Complete your mailing address, including street, city, province, and postal code.
8. Enter your email address and phone numbers (home, cell, and work) in their respective fields.
9. Indicate if you are legally able to work in Canada by selecting 'Yes' or 'No'.
10. Specify your availability for extra work by circling all applicable options: ‘Anytime’, ‘Available weekends’, and ‘Available with notice’.
11. Provide your height and weight in the designated areas to help casting agents assess suitability.
12. Fill in your hair color and eye color for identification purposes in casting.
13. For wardrobe information, enter your sizes for neck, jacket, waist, inseam, and shoe if you are male; and bust, waist, hip, dress size, and shoe if you are female.
14. Indicate if you have three clothing options for casual and business settings, as well as whether you own a tuxedo or evening gown by circling 'Yes' or 'No'.
15. Describe any special skills, hobbies, or talents in the provided area, ensuring to highlight any gymnastic experience.
16. List your current and past occupations in the respective section.
17. Indicate if you have previous extra work experience by circling 'Yes' or 'No', and mention the productions you worked on if applicable.
18. Confirm if you have a valid driver's license and access to a car, including whether it is automatic or standard.
19. Fill in the year, make, and model of your car if you have access to one.
20. Date and sign the form at the end, confirming that all information you provided is accurate.
21. Lastly, attach a current solo photo measuring at least 4 x 6 inches, as required.
