- Submit your completed Caseload, Workload, and Students to Be Contracted Out Forms to your OT/PT Supervisor via email by the specified deadline. Retain signed copies from your school administrators for your personal records.
- Regularly update these forms and resubmit to your OT/PT Supervisor on the designated review dates: January 20, 2017, and June 2, 2017. Submit further updates as changes occur.
