- Specify the barring direction. Choose from 'From' or 'Both' for each entry.
- Fill in the subscriber's requested alteration date, selecting 'Now' if applicable. Add the name and signature of the person making the request, noting that all IMNs linked to a forward ID default to de-activation unless otherwise indicated.
- Once all fields are completed, save your changes and prepare to fax or scan and email the finished form to the designated contact at Fisheries Queensland – VMS Unit.
