How to fill out the Super Choice Form online

Follow the steps to successfully complete the form online

  1. Click the ‘Get Form’ button to obtain the Super Choice Form and open it in your editor.
  2. In section A, as the employee, complete your personal details, including your name, employee identification number, and your tax file number (TFN). This information is essential for your employer to process your super contributions efficiently.
  3. In section A, you will select your preferred super fund. Choose either the APRA fund or RSA by completing items 3 and 5 or opting for a self-managed super fund (SMSF) by completing items 4 and 5. Make sure to attach the required documentation confirming the status of the fund you are nominating.
  4. Once section A is complete, return the form to your employer. Ensure that you have attached any necessary supporting documents for your chosen super fund.
  5. The employer should complete section B by filling in details about their nominated super fund before handing the form back to you. This section must be completed before the employee receives the form.
  6. After receiving the form back from you, the employer will complete section C. This section documents the acceptance of your choice and must be retained for their records for five years.
  7. Finally, the employer must ensure contributions are made according to your chosen super fund in compliance with applicable timelines.

Super choice gives employees the option to swap out of the default MySuper product or fund that was chosen for them and go to another MySuper product or fund. But only if you want to. The point is you have a choice. But equally it doesn't mean employees can choose any super fund they want either.

If you want to make sure your employer is paying into a particular fund, it's important that you complete this form and return it to your employer. You can complete an online version. To do this, log in to ATO online services through myGov and select 'new employment' from the Employment menu.

Default products (known as MySuper products) are those chosen by your employer, while non-default products, known as 'Choice' super products are those that you choose yourself. If you don't make an active decision about where to put your super, you will be placed in the fund your employer nominates.

What is the superannuation standard choice form? The superannuation standard choice form allows you to nominate the super fund that your employer will make contributions into on your behalf. Currently, the super guarantee requires your employer to contribute a minimum of 9.5% of your eligible income to your super.
