The GAD-7 score is calculated by assigning scores of 0, 1, 2, and 3, to the response categories of “not at all,” “several days,” “more than half the days,” and “nearly every day,” respectively, and then adding together the scores for the seven questions. GAD-7 total score for the seven items ranges from 0 to 21.

Score 0-4: Minimal Anxiety. Score 5-9: Mild Anxiety. Score 10-14: Moderate Anxiety. Score greater than 15: Severe Anxiety.

Usage. The copyright is held by Pfizer Inc. but the questionnaire is free to use. The questionnaire has been validated for use as a screening tool and severity measure [1,2], in primary care [3] and in general populations [4].

The GAD-7 represents an anxiety measure based on seven items which are scored from zero to three. The whole scale score can range from 0 to 21 and cut-off scores for mild, moderate and severe anxiety symptoms are 5, 10 and 15 respectively [13]. At the cut-off score of 10 both sensitivity as well as specificity exceed .

The GAD-7 items include: 1) nervousness; 2) inability to stop worrying; 3) excessive worry; 4) restlessness; 5) difficulty in relaxing; 6) easy irritation; and 7) fear of something awful happening.

15–21 Severe This individual's symptoms of anxiety likely warrant active treatment. This individual is likely to be diagnosed with an anxiety or related disorder. Further assessment (including diagnostic interview and mental status examination) and/or referral to a mental health professional is recommended.

Scoring is easily accomplished by summing scores for items. The total score ranges from 0–63. The following guidelines are recommended for the interpretation of scores: 0–9, normal or no anxiety; 10–18, mild to moderate anxiety; 19–29, moderate to severe anxiety; and 30–63, severe anxiety.

This led to the development of GAD-2, which is the shorter version of GAD-7 and uses only the first two questions, which represent the core anxiety symptoms (Tables ​3-​ 4). It tries to highlight the important components which are present regardless of the underlying specific diagnosis.
