- Fill out the allotted seats table by indicating the college codes, college names, universities, and confirmation status for each seat, marking 'Yes' or 'No' as applicable.
- Clearly state the name of the college you wish to join and its corresponding college code and university. Ensure that this information is filled out correctly.
- Conclude the form with a formal request by stating your intent to cancel the already confirmed seat and provide the name of that college and its college code.
- Sign the document in the designated area as a confirmation of your request, and also leave the date of signing.
- The Principal of the college you are joining must sign and stamp the form, indicating their approval of your request.
- Please ensure that the Principal emails a scanned copy of this letter to the provided email address with the subject line formatted as 'CANCEL : (Type Application Number)'.
- Once the form is completed, save your changes, and you may choose to download, print, or share the form as needed.
