Follow the steps to complete your direct deposit application or change form.

1. Click the ‘Get Form’ button to access the form and open it in your digital editor.
2. Identify your status by selecting whether you are a union associate or a non-union associate. Complete Section A or B as appropriate.
3. In the 'Employee Name' section, provide your last name, first name, and middle initial.
4. Enter your employee number, which is the 6-digit number (EZ Time ID#) found on the back of your Montefiore ID or pay stub.
5. If you wish to cancel your direct deposit, authorize this by signing in Section A and providing the date.
6. If you are a union associate and wish to request paper direct deposit statements, proceed to Section B, provide your signature, and the date.
7. Once you have filled out all relevant sections and signed where indicated, review your form for accuracy.
8. Finally, save your changes, download a copy of the form, and share it as needed. Completed forms can be emailed to MyPay@montefiore.org or faxed to (914) 349-8385.
