Follow the steps to complete your Telguard registration.

1. Click ‘Get Form’ button to obtain the registration form and open it for editing.
2. Enter the Telguard digital serial number in the specified field at the top of the form.
3. Select whether the registration is for residential or commercial service by checking the appropriate box.
4. Provide your subscriber information. Fill in your last name, first name, middle initial (if applicable), street address, city, state, and zip code in the respective fields.
5. Next, fill in the dealer information. You will need to provide the dealer account number, dealer company name, street address, city, state, and zip code.
6. Enter the contact person's information. This includes their last name, first name, phone number, fax number, and email address.
7. Indicate how the central station will obtain the receiver and subscriber account numbers by selecting the appropriate option.
8. Provide the primary and secondary receiver numbers, along with the central station account code.
9. Choose the alarm format by checking the relevant box for pass only, fail only, or both.
10. Select the rate plan that best suits your needs by checking the option for residential test signals and normal alarms.
11. For test schedules, select the day of the month or day of the week as required.
12. At the end of the form, the dealer must acknowledge acceptance of the Telular Terms and Conditions. This includes providing a signature, printed name, and date of the signing.
13. Once all information is accurately filled out, save changes to the document. You may also choose to download, print, or share the form as needed.
