6. Identify an emergency contact by providing their name and relationship to the camper, along with a phone number for emergencies.
7. In the physical waiver section, confirm that the camper has had a physical within the past year by signing your name and indicating the camper's name.
8. Review the camp information section and check the camp(s) that your child will attend. Make sure to note the total due for each camp attended.
9. Finalize your registration by making a check payable to Jennies Volleyball Camps and returning it with the payment to the specified address.
10. After completing the form, save your changes, and you may choose to download, print, or share the form as necessary.
