- Fill out section 5 regarding nationality and place of birth for your parents and spouse.
- In section 6, record your date of birth and current age, as well as your age at the time of SSC/Matriculation.
- Complete section 7 by detailing your place of birth and the district and state to which you belong.
- Provide your religion and information about whether you belong to scheduled caste, scheduled tribe, or backward classes in section 8.
- In section 9, list your educational qualifications. Provide information about your schooling from age 15 onwards, including certification and details about study methods.
- Section 10 requires you to fill in any employment history, detailing the designation, time period, and address of each employer. Ensure supporting documents are attached.
- In section 11, disclose any legal issues, such as arrests or convictions. Provide full particulars if applicable.
- In section 12, note the names and addresses of two responsible persons who can vouch for your character; they should not be blood relatives.
- Lastly, complete the declaration section. Ensure you sign and date the form, confirming the accuracy of all information provided.
