- Input your MERS® System Org ID, which is unique to your organization.
- Select the membership type by checking the appropriate box, ensuring only one is checked. Membership types include Investor Patron, Lite, General, Servicing Patron, and Subservicer.
- Complete the relevant pages specified for your membership type. For example, Investor Patrons should stop at page 5, while Lite members will complete up to page 11.

- Submit your completed QA Plan via email to MERSCORP Holdings at compliance@mersinc.org.
