4. Indicate whether you have health or accident insurance by selecting YES or NO. If YES, provide the name of the Insurance Company.
5. Fill out the Emergency Contacts section by providing details for your Primary Contact Person and Alternative Contact Person, along with the Physician's name and contact information. Make sure to include the addresses as well.
6. After completing Parts A and B, you may need to provide additional medical history if you are 18 years or older under Part B: Medical Declaration. Fill in any relevant medical conditions and history by checking the appropriate boxes and providing detailed information.
7. For Part C: Acknowledgement of Risk & Consent, read the statement thoroughly. Sign and date the form to confirm your agreement to the terms outlined.
8. Complete Part D: Medical Fitness Assessment, if required, by bringing the form to a physician for review. The doctor will assess and complete the required information.
