How to fill out the Quickdash online
The Quickdash is a tool designed to assess the impact of arm, shoulder, or hand problems on daily activities and work performance. This guide provides detailed instructions to help you navigate the form with ease.
Follow the steps to complete the Quickdash effectively.
- Press the ‘Get Form’ button to access the Quickdash form and open it in your preferred online platform.
- Enter the patient's name in the designated field at the top of the form. This personal information is essential for identification purposes.
- Record the date of filling out the form in the provided space, ensuring that the form is valid and up-to-date.
- For the work module, describe your current job or indicate if you do not work. If you are not employed, you may skip this section.
- Review the difficulty-related questions in the work module and circle the number that best represents your experience with each activity over the past week.
- Calculate your QuickDASH score for the work module by adding the values of your responses, dividing by 4, subtracting 1, and then multiplying by 25.
- For the sports/performing arts module, specify the sport or musical instrument that is most important to you, or skip this section if it does not apply.
- Answer the questions regarding your ability to use your technique and the difficulties faced while playing or practicing your chosen sport or instrument.
- Similar to the work module, calculate your QuickDASH score for the sports/performing arts module by following the same scoring method.
- Finally, make sure to sign and date the form, and then save any changes you have made. You can choose to download, print, or share the completed document as needed.
Complete the Quickdash online to efficiently assess your condition and impact on daily activities.
At least 10 of the 11 items must be completed for a score to be calculated and the scores range from 0 (no disability) to 100 (most severe disability). This score was designed be useful in patients with any musculoskeletal disorder of the upper limb.
