The classic sequences: first molar-central incisor-lateral incisor-first premolar-canine-second premolar-second molar (M1-I1-I2-PM1-C-PM2-M2) in the upper arch and I1-M1-I2-C-PM1-PM2-M2 in the lower arch occurred in only 16% of upper arches and 13% of lower arches.

The first teeth to erupt are the lower central incisors which usually appear around six months of age. The last primary teeth to erupt are the second molars, which generally erupt at approximately 2 years of age.

The first permanent teeth that emerge in the oral cavity are the maxillary and mandibular first molars at around six years of age (six-year molars). They erupt distally to the deciduous second molars. Their eruption is accompanied by or preceded by the exfoliation of the deciduous mandibular central incisors.

Calcification of the central incisor begins at approximately 14 weeks in utero, with the maxillary central incisor slightly preceding the mandibular central incisor. The initial calcification of the lateral incisor occurs at 16 weeks and that of the canine at 17 weeks.

Primary (baby) teeth usually start coming in at the age of 6 months, and permanent teeth usually start coming in at about 6 years.

The primary maxillary canines erupt at about 19 (16 to 22) months (Figure 2-8), and the mandibular canines erupt at 20 (17 to 23) months.

Dental Basics The first permanent tooth to erupt is the first molar at approximately age 6 years. It erupts distal to the primary second molar. The remaining permanent teeth erupt in the following order: Mandibular central incisors, 6 years.

Delayed eruption of maxillary permanent incisor teeth usually occurs in the early mixed dentition and is often observed at age of 7–9 years.
