In the acute phase of traumatic hemorrhagic shock, the therapeutic priority is to stop the bleeding. As long as this bleeding is not controlled, the physician must manage fluid resuscitation, vasopressors, and blood transfusion to prevent or treat acute coagulopathy of trauma.

Advanced Trauma Life Support (ATLS), developed by the American College of Surgeons, promotes the primary survey sequence as airway, breathing, circulation, disability, exposure (ABCDE). Once the airway is secured or maintained by the patient, breathing and ventilation should be assessed.

The ABCs of trauma assessment-airway, breathing/oxygenation and circulation-represent the first steps in determining the extent of injury and patient care priorities; however, other factors must also be considered to provide effective and timely care.

There are 3 priority categories in which the trauma team can be activated: Level 1/Geriatric Level 1, Level 2 or Consult. In general, patients who meet Level I or II activation criteria should be evaluated in the Emergency Department.

Massive hemorrhage can be addressed by the four Ds: Detect: find the source of the bleeding. Direct pressure: hold pressure on the source of the bleeding until the clot forms. Devices: if necessary, use equipment such as tourniquets, hemostatic gauze and pressure bandages to supplement direct pressure.

Stop the bleeding. Cover the wound with sterile gauze or a clean cloth. Press on it firmly with the palm of your hand until bleeding stops. But don't press on an eye injury or embedded object. Don't press on a head wound if you suspect a skull fracture.

Initial management after severe injury involves control of hemorrhage, administration of intravenous isotonic crystalloid fluids, and blood transfusion, if needed, which immediately improves effective circulating volume.
