8. Complete the Mentor Liability Release and Position Summary, which outlines your responsibilities as a mentor.
9. Sign the application acknowledging your understanding of the mentoring agreement and provide the date.
10. Authorize the disclosure of your criminal record by signing the applicable section at the end of the application.
11. Once all sections are filled out accurately, save your changes, and consider downloading, printing, or sharing the form as necessary to submit it.
