8. If your billing address is different from your service address, provide the required billing information in the spaces provided.
9. Complete the sections for your License/ID number, Social Security number, date of birth, work phone, and employer details.
10. Read the disclaimer about equipment rental and service disconnection for non-payment, then sign and date the application.
11. Once you have completed the form, save your changes, and choose to download, print, or share the document as needed.
