9. If you are an Active member, list a minimum of eight dances taught in the past year, ensuring that four are within the state of Kansas. Use attachments if required.
10. Document the details of classes you taught, including the club name, city, average number of dancers, dancing level, and frequency of sessions.
11. Affirm your ASCAP/BMI license status as an Active member by signing or typing your name and providing the date.
12. For new Active members or upgrades, state how many years you have been calling/cueing/prompting and provide contact names of individuals who can vouch for your qualifications.
13. Optionally, add any additional information regarding your qualifications on the back of the form and check the box provided.
14. Ensure the form is completed year-to-year and submitted with the appropriate dues to the KSDCA secretary prior to the January 31 deadline.
