Follow the steps to accurately complete your Mas Time Sheets

- Enter the facility name where you worked in the designated space.
- Starting with Sunday, input the date of your shift in the DATE section.
- Record the time you started working in the TIME IN field for the corresponding day.
- If you took a lunch break, specify the duration in the LUNCH section, using the nearest quarter hour as a guide.
- Indicate the time you finished your shift in the TIME OUT field.
- Calculate your total hours worked, rounding to the nearest quarter hour, and enter this in the TOTAL HOURS field.
- You need to gather the initials from the facility representative in the FACILITY INITIALS section for each day worked.
- At the bottom of the time sheet, sign and date the Employee Verification to certify the accuracy of the hours reported.
- Have the authorized representative from the facility sign and date the Client Verification section to confirm the hours worked and approval for payment.
- Once completed, save your changes, and ensure to fax the time sheets by 12:00 noon on Monday to 866-611-6660.
