7. Proceed to Section D for the disability assessment. Indicate whether you consider yourself a person with a disability and provide details about the type or class of disability, if applicable.
8. In Section E, specify the mode of payment for your application fee. Options include banking slip, banker’s cheque, money order, or M-Pesa. Provide the corresponding reference number and the amount paid.
9. Finally, complete Section F by reading the applicant's declaration. Confirm that the information you provided is accurate by signing and dating the form.
10. Once you have filled out the entire form, review all the information for any errors. You can then save changes, download the form, print it, or share it as needed.
