The FLACC scale was originally designed and validated for use in infants and children aged 2 months to 7 years to measure postoperative pain. The original instructions for use recommended observing the child for 1–5 min and matching the observed behaviours to those described in the scale for each item.

The NCCPC-PV is a 27-item scale (range 0 to 81) and uses a Likert-like format “0-not at all,” “1-just a little,” “2-fairly often,” or “3-very often” during 10-minute observation. A prior study of the NCCPC-PV indicated moderate pain is a level at which most children would require pain medication.

The Non-Communicating Children's Pain Checklist-Postoperative version (NCCPC-PV) is a pain assessment tool specifically designed for children between 3 and 18 years of age with ID, which has been developed from semi-structured interviews with parents of children with severe neurological impairment.

The FLACC scale was used by the parents and the nurses to evaluate pain levels in children up to 4 years of age (Table 1), and the NRS was used by patients, parents and nurses for children aged 4 years and older.

Faces Pain Scale Revised (FPS-R) This scale is often used with children ages 4-8 years old. It can also be used for older children who have trouble using the number scale.

Remember that for children it often useful to substitute the terms “ouch” or “hurt” for the word “pain”. The numeric rating scale may be categorized into no pain (0), mild pain (1-3), moderate pain (4-6), and severe pain, (7-10). These categories have been used in the past to indicate whether an opioid is indicated.
