Follow the steps to complete your complaint form online

1. Click ‘Get Form’ button to obtain the complaint form and open it in your preferred editor.
2. Fill in the complainant's details section. Provide your full name, address, and contact information, including your telephone number and email address.
3. In the details of the banking institution/payment instrument issuer section, specify the name and address of the institution you are complaining about. Include the branch concerned and the name of the person you have already contacted.
4. In the accounts details section, select the appropriate box to indicate whether your account is Islamic or Conventional. Enter your account number and select the type of account you hold.
5. Tick the type of complaint you wish to file from the provided options. This could include loan issues, fees and charges, customer service problems, and others.
6. Clearly explain the details of your complaint in the designated area. Attach photocopies of any relevant documents that support your claim, such as loan agreements or police reports.
7. Review your form to ensure that all fields are completed accurately. Confirm that all necessary documents are attached.
8. Date and sign the form, acknowledging that the information provided is true and given voluntarily.
9. Submit the completed complaint form along with the attached documents to the Consumer and Market Conduct Department at Bank Negara Malaysia.
