- Fill out your residential address details including house number, street, and district. Specify your Mandal or Municipality name and Village/Town/City name along with your mobile number and email address.
- In the identification details section, provide your ration card number and any identification marks. Ensure you fill this section accurately.
- Complete the pension office details, specifying the district from where you receive your pension, including the STO/APPO name and code.
- Gather required attachments. Scan and upload the necessary documents including your Pension Payment Order, passport-size photo, Aadhaar card/receipt, and disability certificate if applicable.
- For dependent family members, fill out their details including relationship, name, date of birth, and any disability, using similar criteria as your own information.
- Review all entered information for accuracy. Once all sections are complete, you can save your changes, download a copy, print, or share the form as needed.
