I made a mistake or misspelled something on the form. How to change it? You cannot change it online, BUT you can correct it at the DFA office on your appointment schedule.

What forms do I use for passport renewal? If you are eligible, you may use form DS-82 and send in your renewal application by mail. If you are not eligible to send in the application by mail, you must use form DS-11 and apply in person. If you are making a change or correction, you may use form DS-5504.

Submit the following by mail: Form DS-5504 (find mailing instructions on the form) Your most recent U.S. passport. Your original or certified name change document, such as a marriage certificate, divorce decree, or court order.

The only way to remedy a mistake on an online passport application form after submission is by way of a written submission sent via post to the passport office. It must outline the error and its remedy and must be signed and dated by the individual named on the declaration form.

You can fill out the form using the online form filler of the Department of State website (https://pptform.state.gov/) or print it out and fill it out by hand using a black pen.

To correct a data or printing error, submit the following by mail: Form DS-5504 (find mailing instructions on the form) Your current passport. One color photo. Evidence of the error (e.g. your U.S. birth certificate showing the correct spelling of your last name). You will not have to submit any fees.

Follow the instructions on Form DS-82 if you are renewing, or Form DS-5504 if you are changing your name within one year of a passport being issued. Collect your supporting documents. Be sure to include the $60 expedite fee in addition to the normal application fee.

You qualify to have your passport replaced using a DS-5504 form; this form can only be used to replace limited validity passports (emergency passports) and passports that had an error when they were printed within a year of its issue date.
