Form 410 is the form used by creditors to file a proof of claim. In some cases, a bankruptcy judge may accept an informal proof of claim. This must be a written document filed with the bankruptcy court, and it must make a demand against the debtor's bankruptcy estate.

A notice of transfer of claim is what creditors file when they buy debts from other creditors holding debts owed by a debtor in bankruptcy. For example, if the debtor lists a debt owed to Bank A in a chapter 13 bankruptcy petition, then Bank A will file a proof of claim with the bankruptcy court.

A proof of claim form is the official bankruptcy form a creditor must file before getting paid in a bankruptcy case. The proof of claim form tells the bankruptcy trustee the type of claim the creditor asserts and how much the filer owes the creditor.

The proof of claim must be signed by the individual completing the form. The signature of the claimant must be witnessed. A complete address must be provided including the postal code where all notices and correspondence are to be forwarded.

A written statement filed in a bankruptcy case setting forth a creditor's claim is called a proof of claim. A proof of claim should include a copy of any documentation giving rise to the claim as well as any evidence in support of the claim, such as evidence of secured status if the claim is secured.

This “proof of claim” is the vehicle for a creditor to assert its claims against the debtor in the bankruptcy proceeding (the automatic stay of bankruptcy prohibits creditors from asserting claims against the debtor outside of the bankruptcy proceeding and from exercising control over any of the debtor's property).

A claim is "transferred other than for security" if it is not transferred for the purpose of providing collateral. The buyer should file a proof of claim as soon as possible. Proofs of claim in Chapter 11 cases must be filed before a court- established bar date to be allowed.
