- In section 2b, indicate whether you wish to request copies of records, similarly selecting 'ALL' or 'UNLAPSED' and the certification option as needed.
- Complete section 2c if you are requesting specific records by record number.
- For additional services, complete section 3 if your filing office offers specific services apart from those on the form.
- In section 4, outline your delivery instructions, whether you wish to have the information mailed or picked up. Specify any alternative delivery method required.
- After carefully reviewing all the information you provided, ensure the form is complete and accurate, as mistakes can lead to legal issues.
