Follow the steps to fill out the 7 Eleven sponsorship request form

1. Press the ‘Get Form’ button to access the sponsorship request form and open it in your preferred editing tool.
2. Begin by entering your department name if you are an internal department or employee. Next, fill in your name as the requester, provide a justification for the sponsorship, and include your signature along with the date of the request.
3. For organizations requesting sponsorship, input the name of your organization along with the title of the contact person and their complete address, including street, city, state, and zip code. Don’t forget to fill out the phone number and email address for further communication.
4. Specify the name of your organization that is requesting sponsorship. Then, state the name of the opportunity or event related to this request, along with the NGO or organization registration number, event date(s), location, and the amount of sponsorship funding you are requesting, either in cash or kind.
5. Describe the mission of your organization clearly, and provide a brief explanation of the sponsorship opportunity or event you are organizing. Highlight the audience demographics and state the expected attendance numbers.
6. Indicate whether 7 Eleven Malaysia has sponsored your event or organization in the past, and if so, include the date of the last sponsorship.
7. Explain how this sponsorship opportunity or event will be beneficial to both 7 Eleven Malaysia and the community. If your organization has board representation or any affiliation with a Berjaya Corporation staff member, provide details regarding that affiliation.
8. Include any additional information that may support your sponsorship request as needed.
9. To finalize your request, ensure that you have completed the sponsorship request form as well as the letter of request on your organization’s letterhead. You will also need to provide ROS, ROC, SSM documentation, and a list of the organization’s board of directors or event committee members.
