6. Next, read and complete the Applicant’s Declaration by filling in your name.
7. In the Guarantor’s Attestation section, the guarantor must also fill in their details and sign the form.
8. After that, in the HOD Recommendation section, the Head of Department will need to fill in their recommendation and sign.
9. The final section involves confirmation by the Directorate of Finance. This includes their signature, date, and official stamp to confirm the applicant's enrollment and salary deductions.
10. Review all filled parts of the form for accuracy, and upon completion, you can save changes, download, or print the form as needed.
