The Functional Behavioral Assessment (FBA) Data Record Form is a comprehensive data collection and synthesis tool designed to assist the professionals in determining what function a specific behavior serves for a student. A FBA is the foundation on which a behavioral intervention plan may be developed.

There are three main categories of functional assessment approaches—indirect (e.g., questionnaires, rating scales), observational, and experimental/functional analysis.

An FBA must be conducted: Whenever the Individualized Education Program (IEP) team (1) determines that a student's behavior is interfering with his/her learning or the learning of others, and (2) requires additional information to provide appropriate educational programming.

All requests need to be done in writing. A simple email will do. “Dear IEP team, I am requesting that my child receive an FBA.... have experience conducting FBA. they have been properly trained and certified (varies by state) they have experience with the behavior challenges/diagnosis of your child.

Who Can Perform an FBA? More than one type of person can perform an FBA. A person trained in the specific type of data collection and analysis should conduct the interviews and observations. Examples of people who can do an FBA are school psychologists, special education teachers, assistant principals, and principals.

From a clinical standpoint, functional assessments should be conducted when the student's behavior interferes with their own learning or the learning of others, presents a danger to self or others, or the behavior results in suspension or interim placement in an alternative setting approaching 10 total days.

Both FBA and BIP documents must be created OR updated at least once a year. Since this document is dealing with current problematic behaviors, it makes no sense to refer to very old data. 6.

If a student's behavior is caused by or had a direct relationship to their disability, then an FBA is required. It's also required when law enforcement, weapons, drugs, or serious injury are involved. Schools often do FBAs to evaluate risk for students with serious behavior issues.

In a functional assessment, the purpose of an individual's behaviours is explored by collecting data specific to the individual, their relationships and their environments. Using this information, the specialist is able to develop appropriate teaching and/or intervention strategies that are specific to the individual.
