  11. Complete the section on whether you have an active appeal of a DCF investigation by checking ‘Yes’ or ‘No’.
  12. Finally, sign and date the form where indicated. Remember that this authorization will expire 180 days after the date of your signature. Ensure there are no blank spaces and specify 'N/A' for any non-applicable items.
