Filling out the Extended School Year (ESY) Services Eligibility Worksheet is an important step in ensuring that students receive the necessary support during breaks in their education. This guide will provide a clear, step-by-step breakdown of how to complete the worksheet online, ensuring that you can navigate each section with confidence.

  1. To begin, press the ‘Get Form’ button to access the form and open it within your preferred editing platform.
  2. Input the student’s name in the appropriate field labeled 'Student Name'. This should be the full legal name of the student for accurate identification.
  3. Enter the Student ID Number in the designated field. This should correspond with the student's official identification used within the educational institution.
  4. Specify the LEA (Local Education Agency) of enrollment for the student. This indicates where the student is currently enrolled and is crucial for determining eligibility.
  5. Begin evaluating Criterion 1 by circling either 'NO' or 'YES' to indicate if one or more of the student’s critical skills will be jeopardized by the break in service. If 'YES', provide details regarding which critical skill(s) are affected.
  6. Proceed to Criterion 2 and assess whether there is a likelihood of significant regression in the identified critical skills. Again, select 'NO' or 'YES' and describe the anticipated degree of regression if applicable.
  7. Next, evaluate Criterion 3. Indicate whether the student can recoup the identified critical skill(s) with re-teaching in a reasonable amount of time by selecting 'NO' or 'YES.'
  8. After completing the criteria evaluation, document the IEP team's determination regarding the student’s eligibility for ESY services by selecting 'NO' or 'YES'.
  9. Finally, record the date of determination in the format ______/______/20____, then ensure that you follow the next steps by entering the team’s decision in the ESY section of the student’s IEP in SEDS.
  10. Print out the cover sheet located on the Documents Tab in SEDS and fax both documents into SEDS. Complete the services and goal portion of the ESY section in the student’s IEP.
