How are the KOOS scores interpreted? The score is a percentage score from 0 to 100, 0 representing extreme problems and 100 representing no problems. This direction, 100 indicating no problems, is common in orthopaedic instruments and generic measures like the SF-36.

KOOS is developed as an instrument to assess the patients opinion about their knee and associated problems. KOOS is intended to be used for knee injury that can result in post traumatic osteoarthritis (OA); i.e. ACL (anterior cruciate ligament) injury, meniscus injury, chondral injury, etc.

Each item is scored 0-5 with 5 indicating “no difficulty” and 0 representing “unable to perform”. The highest possible score is 70. The scores of all items are summed, divided by 70, then multiplied by 100 to give an overall ADLS percent rating. Higher percentages reflect higher levels of functional ability.

KOOS, JR is scored by summing the raw response (range 0-28) and then converting it to an interval score using the table provided below. The interval score ranges from 0 to 100 where 0 represents total knee disability and 100 represents perfect knee health.

The five patient-relevant subscales of KOOS are scored separately: Pain (nine items) Symptoms (seven items) ADL Function (17 items) Sport and Recreation Function (five items) Quality of Life (four items)

There is some evidence that the KOOS subscales demonstrate sufficient unidimensionality, but this requires confirmation. Although measurement error requires further evaluation, the minimal detectable change for KOOS subscales ranges from 14.3 to 19.6 for younger individuals, and ≥20 for older individuals.

The KOOS is self-administered and assesses five outcomes: pain, symptoms, activities of daily living, sport and recreation function, and knee-related quality of life. The KOOS meets basic criteria of outcome measures and can be used to evaluate the course of knee injury and treatment outcome.
