7. Document your educational attainment details, starting with your secondary education. Include the year graduated, GPA, name and location of the school, as well as any academic honors and awards received.
8. If applicable, provide details about your college education, including year graduated, latest semester grade, name and location of the school, course, academic honors, and any other awards.
9. Describe how you learned about the Philippine Military Academy.
10. Review the certification section, affirming that all information provided is true and correct. Your signature and thumbprints will be required here.
11. After completing the form, save changes. Finally, mail your application form to the Office of Cadet Admission at the Philippine Military Academy together with the required documents.
