A Qualified Independent Contractor (QIC) is an independent entity with which Medicare contracts to handle the reconsideration level of an Original Medicare (Part A or Part B) appeal.

The Social Security Act (the Act) establishes five levels to the Medicare appeals process: redetermination, reconsideration, Administrative Law Judge hearing, Medicare Appeals Council review, and judicial review in U.S. District Court.

You must send your request to the QIC that will handle your reconsideration. The QIC's address is listed on the redetermination notice. You can submit additional information or evidence after the reconsideration request has been filed, but it may take longer for the QIC to make a decision.

How to File An Appeal Via secure fax to 585-425-5292. Via mail to the following address: Maximus, Inc. Medicare Managed Care & PACE Reconsideration Project. 3750 Monroe Avenue, Suite 702. Pittsford, NY 14534-1302.

Appeals Level 2: Qualified Independent Contractor (QIC) Reconsideration. A QIC is an independent contractor that didn't take part in the level 1 decision. The QIC will review your request for a reconsideration and make a decision.

The Medicare, Medicaid, and SCHIP Benefits Improvement and Protection Act (BIPA) of 2000 included provisions aimed at improving the Medicare appeals process. Part of these provisions mandate that all second-level appeals, also known as reconsiderations, be conducted by Qualified Independent Contractors (QICs).
